Job Description: Community Health Workers are an important resource for communities, connecting individuals and families to services and information they need.
Community Health Workers (CHWs) are frontline public health workers who are trusted members of the community served. This trusting relationship enables CHWs to serve as a liaison between health & social services to facilitate access to services and improve the quality and cultural competence of service delivery.
CHWs also build individual and community capacity by increasing health knowledge and self-sufficiency through various activities such as outreach, community education, informal counseling, social support, and advocacy.
Community Health Workers are increasingly recognized for their powerful potential role in improving the health care system. Recent studies have demonstrated that CHWs can help to reduce costs and improve care – key goals of most states’ health care priorities.
Community-embedded and highly accessible, pharmacies are uniquely positioned to connect individuals to local community and social resources and thereby address SDoH. We are seeking people with CHW credentials to assist with the following activities in our pharmacies…
Resolve gaps in care
Identifying alternative cost savings opportunities
Collecting health information
Reporting health information
Supporting partnership development and increased interdependence between the pharmacies, local community organizations, and the public health sector.
Receiving referrals & connecting OUR patient to LOCAL services that meet OUR patient’s individualized needs
Help patients and their families navigate the health care system, access LOCAL community services & non-local resources, as well as promote the adoption of healthy behaviors
Facilitate a LOCAL and ACCESSABLE destination for soft handoffs for patients in transition and/or in need of services beyond standard prescription dispensing
Serve as a LOCAL liaison for payer and provider partners and become a conduit for information flow and service delivery
Coordinate refills, care, inventory, & delivery
Build peer relationships important for patient retention
Use relationship to engage in enhanced services (immunizations, etc.)
Strategic assignment of patients (high margin, statin, rasa, DM, etc.)
Assist with medication synchronization, packaging, med optimization, transition of care, etc.
Assist with Medicare Part D annual enrollment reviews
Assist with Medication Therapy Management and Complete Medication Review scheduling and follow up
Increase workflow efficiencies within and among pharmacy teams
Salary/ Rate of Pay : $13-$15 / hr or depending on additional credentials (if possessed)

Requirements: High School Diploma / HSE
Completed DHSS-approved CHW Standard Training Program
Current CHW credential issued by the Missouri Credentialing Board
Pharmacy technician registration (required upon hiring which includes a background check)
