Program Facilitator

Salary: $24.00/HR

Job Description: 1. Assessment:
Conducting comprehensive assessments of tobacco use history, patterns, and readiness to quit. This may involve evaluating the individual's smoking behavior, nicotine dependence, previous quit attempts, and any co-existing medical conditions.
2. Treatment Planning:
Developing personalized quit plans tailored to to each individual's needs and preferences. This will include setting quit dates, identifying triggers, and discussing strategies for coping with cravings and withdrawal symptoms.
3. Education and Counseling:
Providing evidence-based information and counseling to support tobacco cessation efforts. This will involve educating individuals about the health risks of tobacco use, the benefits of quitting, and the available cessation resources and medications.
4. Behavioral Interventions:
Implementing behavioral interventions, such as cognitive-behavioral therapy (CBT), motivational interviewing (MI), and relapse prevention strategies, to help individuals modify their smoking behavior and build healthier habits.
5. Pharmacotherapy Management: Assessing the appropriateness of pharmacotherapy (e.g. nicotine replacement therapy) for nicotine cessation and providing guidance on the proper use, dosage, dosage and potential side effects of these medications.
6. Support and Follow-up:
Offering ongoing support and follow-up to individuals during their quit attempts. This may involve regular check-ins, support groups, or referrals to additional resources as needed.
7. Documentation and Evaluation:
Maintaining accurate records of client assessments, treatment plans, interventions, and outcomes. Monitoring progress towards cessation goals and evaluating the effectiveness of interventions over time.
