Job Description: JOB SUMMARY

In accordance with agency Mission, Vision, Values and Goals, the Counselor/Therapist, provides ongoing individual and group counseling to assigned population and provides group education as requested and documents all interactions to reflect medical necessity. The Counselor/Therapist interacts regularly with treatment team members and other treatment providers to ensure quality care to the individuals being served. Counselors/Therapists may also provide crisis intervention, case management, wellness planning and assessments.

PRIMARY FUNCTIONS
Provides therapy/counseling services to the assigned caseload of clients. Therapy services may include individual and family therapy as clinically appropriate.
Provides group therapy and/or group education as assigned.
Completes all necessary documentation as defined by program, agency and DMH/Medicaid standards. Ensures that all documentation is entered into the electronic medical record (EMR) in a timely fashion according to agency policy.
Ensures that wellness plans and any clinically appropriate reviews are completed for all assigned clients. Wellness plans should be completed with the client whenever clinically appropriate and with a person-centered approach, documented in the EMR according to agency policy and meet all quality expectations and guidelines.
Provides clinical services using evidenced based practices. Evidenced based practices that are utilized should be within the scope of knowledge and training of the therapist/counselor.
Serves as a vital multi-disciplinary treatment team member, and facilitates program, departmental and agency communication/operations through required attendance at any treatment team meetings, department meetings, supervisory meetings, and other meetings as assigned.
Assesses clients for potential risk and ensures appropriate documentation and communication with the treatment team per risk management protocol. Ensures the client’s involvement in the development of an appropriate supportive care plan as clinically appropriate.
Responds to all phone calls and client requests within 24 hours and if unable to do so will engage the assistance of supervisor to provide assistance.
Completes initial assessments and all other necessary and required documentation for clients entering services, as back up to Intake and Crisis Clinician as needed.
Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.
QUALIFICATIONS
Education: Master’s degree in counseling or social work required.
Experience: A minimum of 2 years of experience is preferred.
Licenses, Certifications, or Registrations:
Outpatient Department –current Missouri License: provisionally licensed, or fully licensed LPC or LCSW preferred.
Substance Use Department – must be recognized, registered, certified or licensed. Current Missouri License: provisionally licensed, or fully licensed LPC or LCSW preferred.

REQUIREMENTS FOR POSITION
Maintains standards of agency and privileging policies and procedures.
Must acquire and maintain required training per agency policy and DMH regulations to include documented attendance of professional development/training or education.
DEGREE OF SUPERVISION
Licensed Professional:
Weekly supervision with Outpatient Director or other licensed professional for one (1) month following hire; and monthly supervision thereafter.
Substance Use counselors:
Will meet monthly for individual supervision weekly in group supervision. Any clinical documents needing additional signatures will be reviewed in weekly treatment team meetings by qualified staff.
